Thinking About Booking an Appointment?

Dr. Morar, co-founder of Duber Medical, provides an overview for getting your Medical Marijuana Card. Please check out this video to learn more about the appointment and patient process, as well as some general information and facts about medical marijuana.

Booking Your Appointment

Here is the information you will need to schedule your appointment booked with one of our doctors.

Before you book, you will need:

  • a Valid Form of ID such as
    • Driver’s Licenseor State ID
    • US Passport
    • Birth Certificate
    • Social Security Card
    • Proof of Residence (i.e. utility bill, credit card statement) if your ID does not list an address
  • Medical History and/or Medical Records
  • An email account you readily have access to for appointment confirmation and card activation emails
  • Form of Payment
    • Duber Medical accepts all major credit cards
    • at this time, we cannot accept insurance due to federal laws

Qualifying Conditions

The conditions approved vary by State.

The Qualifying Conditions that will be approved for Medical Marijuana vary greatly by State. It is important to review the local laws and understand the approved conditions. Our Doctors will review your qualifying conditions during your appointment and suggest the medical marijuana products appropriate for your diagnosis.

Before Your Appointment

Below are instructions on preparing for your appointment with one of our doctors.

Once you’ve scheduled your medical marijuana virtual consultation, you will receive a confirmation email from Duber Medical with the date, time and doctor you will be seeing, as well as a Zoom hyperlink. If you are new to Zoom, instructions for how to use Zoom are in the confirmation email.

 

Please login to Zoom a few minutes before your appointment. You will enter a virtual waiting room and the doctor will admit you to the video chat when he or she is ready for you. You will have a virtual consultation to discuss your medical history and condition with the doctor and at the end of the appointment, the doctor will indicate whether or not you qualify for medical marijuana.
