assume the rights and obligations regarding your personal information as described in this Policy. - DISCLOSURE OF INFORMATION TO COMPLETE RECOMMENDATION CERTIFICATION PROCESS
When you register as a patient and request an appointment or consultation with a Duber Medical healthcare provider or
physician, personal health information will be required such as your health condition, medical history, medication
regiment, treatment history, prior use of medical marijuana, as well as other information deemed necessary by the
physician to complete the screening, prequalification, and certification process.
We are committed to HIPPA compliance. HIPPA requires the protection of patient health information from being
disclosed without the consent or knowledge of the individual. If the patient chooses to complete the medical marijuana
recommendation certification process and become a registered user must consent to the following: - If a Duber Medical physician recommends treatment using medical marijuana, Duber Medical must
store identification information and personal health information, and disclose certain information to
your respective state’s reporting system and medical marijuana control program. For example, in Ohio,
the reporting system is the Ohio Automated Rx Reporting System and the Ohio Medical Marijuana
Control Program. States with approved medical marijuana programs have their respective reporting
system and medical marijuana control program, and your information would be disclosed to your
respective state’s program. - During the medical marijuana recommendation certification process, Duber Medical physicians must
enter your driver’s license or other identification information, email, address, phone number, and
qualifying condition into your state’s Medical Marijuana Control Program. - Your driver’s license number, physician recommendation number, and specific instructions for use will
be accessible to dispensary staff to allow you access to the dispensary and aid with choosing the correct
quantity, and/or route of administration. - THIRD PARTY PAYMENT PROCESSOR
Duber Medical uses a third-party payment processor for certain payment services (card acceptance, merchant
settlement, and related services). By making use of some or all of these payment services available through Duber
Medical, you agree to be bound by the applicable payment processor terms and conditions and Privacy Policy. If you
provide us with your credit card information during the payment process, we send it to our payment gateway for
