Booking Your Appointment

Here is the information you will need to schedule your appointment booked with one of our doctors.

Before you book, you will need:

• a Valid Form of ID such as
  • Driver’s License
  • State ID

You must prove your identity when you register with the state of New York Medical Cannabis portal by entering your 9-digit driver’s license or state ID number. You will also be able to use this number for proof of New York State residency or you can upload one of the following:

• Pay stub from within the last two months
• Copy of your current lease agreement
• Mortgage statement from the last two months
• A copy of a utility bill from the last two months
• NYC ID Card
• Official Social Security correspondence showing your current address
• Practitioner/hospital bill from the last two months
• Medical History and/or Medical Records

Before Your Appointment

Below are instructions on preparing for your appointment with one of our doctors.

Once you’ve scheduled and PAID for your medical marijuana virtual consultation, you will receive a confirmation email from Duber Medical with the date, time and doctor you will be seeing, as well as a hyperlink for the video chat. 

Please login to the video chat waiting room a few minutes before your appointment. You will enter a virtual waiting room and the doctor will admit you to the video chat when he or she is ready for you. You will have a virtual consultation to discuss your medical history and condition with the doctor and at the end of the appointment, the doctor will indicate whether or not you qualify for medical marijuana. 

After Your Appoinment

Once you have had your appointment, here is what to expect and when you will receive your certification.

If you qualify, you will receive TWO emails from Duber Medical within a few hours of your appointment. This email will contain:

1. A personal treatment plan customized by your physician
2. A written certification from your physician. You MUST take this certification to any NY dispensary with you, along with your medical marijuana license issued by the state. 

Please follow the instructions in your follow-up email to register or login with the state of New York at MY.NY.gov. 

Once your account is active with the MY.NY.gov state website, please follow the below steps to complete your MMJ certification application:

1. Click on the “Health Applications” tile 
2. Click on the “Medical Cannabis Data Management System (MCDMS)” link
3. Click on the “Start New Registration” button
4. Follow the steps to register or renew your medical marijuana license 

When your registration is approved, a new card will be mailed to you. You will have immediate access to a temporary ID card that is valid for 30 days.

The state of New York no longer charges an annual fee for a medical marijuana card. Patient certifications are good for one year from the date of issuance.

March 5, 2023 – Updates are currently being made to the Medical Cannabis Data Management System (MCDMS) that will support the auto-registration of patients and certain caregivers as outlined in Part 113 Medical Cannabis Regulations that were adopted on February 22, 2023. The “auto-registration” of patients at the time of certification will allow patients to go directly from their health care provider to a medical cannabis dispensary without having to file a registration with the Medical Cannabis Program, increasing access for patients across New York State.

We expect the updates to be completed and deployed to MCDMS later this month.

For Caregivers

IMPORTANT: You must be listed on a patient’s registration as a designated caregiver and that patient registration must be approved before beginning your caregiver application. Please check with your patient to ensure these steps are completed before beginning the below registration.

1. Caregivers should create or login to their account at MY.NY.gov account
2. Click on the “Health Applications” tile
3. Click on the “Medical Cannabis Data Management System (MCDMS)” link
4. Click on the “Start New Registration” button 
5. Follow the steps to register

Caregiver registrations will automatically renew when their patient renews their annual certification. Caregivers do not need to renew their registration annually if the patient maintains their medical marijuana license.

New York Qualifying Conditions

The state of New York no longer has a set list of qualifying medical conditions. You may be eligible for the use of medical cannabis if deemed clinically appropriate by your certifying health care provider.
